The Team
Meet your Doctor
Dr. Sana Siddiqui is a board certified internist and the founder of VitalCare Internal Medicine, a direct primary care practice in Concord, New Hampshire, dedicated to the care of adults with a particular focus on women’s health. She completed her undergraduate studies at the University of Toronto and her internship and residency in internal medicine at Bridgeport Hospital, part of Yale New Haven Health.
After residency, Dr. Siddiqui practiced as a primary care internist with Community Health Services in Napoleon, Ohio, from 2016 to 2019, then returned to New England in 2020 to join Concord Hospital, where she cared for patients until 2024. Her internal medicine training spanned the clinic, the hospital and the nursing home, giving her depth in managing complex adult health conditions that she now brings to every patient relationship.
She opened VitalCare to practice medicine the way she believes it should be practiced, with longer and unhurried visits, direct communication with her patients, and clear membership pricing without insurance billing or copays. Dr. Siddiqui is certified by the American Board of Internal Medicine (ABIM), licensed in New Hampshire, and a member of the American College of Physicians (ACP), and she has presented clinical research at the annual meetings of its Connecticut Chapter.
